Care card is ‘best practice’
 

The first CdLS Care Card should be delivered with this edition of Reaching Out. The card is already being heralded as ‘best practice’ by other groups involved with people with rare disorders.

The card is designed to help doctors across many disciplines understand CdLS and to know how other doctors are treating your family member who is affected by the syndrome.

Team

Effectively the card will work as a ‘virtual team’ for the CdLS patient and will allow doctors to contact each other quickly if they have any cross-treatment queries. You should carry it with you for all medical appointments.

Once you have completed the reverse side, extra sheets are available on the Internet and you can download them to print from our site at: www.cdls.org.uk The Care Card can be found on the Treatment Protocols section of the site.
